At Lavan Dementia Care Services, we specialise in providing compassionate, nurse-led care for individuals living with dementia. Founded in 2022 by registered mental health nurse Jade Lavan, our mission is simple: to offer personalised, dignified, and empowering support to those navigating the dementia journey — and to the families walking alongside them.
Lavan Dementia Care Services is a specialist nurse led community service based in Guisborough that operates a dementia care day service, at home services and independent specialist dementia nurse service.
The dementia care day service is based at Sleights Community Hub, Sleights Court, Guisborough, TS14 6TE. The community hub is a safe space with disabled facilities and access, in a quiet residential area of Guisborough. The dementia care day service is specialist nurse led and open 5 days per week (Monday, Tuesday, Thursday, Friday and Saturday) and the opening hours are 10:00 – 18:00.

Independent Specialist Dementia Nurse Service
Led by our founder Jade Lavan, a Registered Mental Health Nurse, this service brings expert clinical support directly to families and care providers.
